The Care Coordinator plays a key role in ensuring the smooth operation of care delivery. You will be responsible for scheduling, staff deployment, customer liaison, and supporting compliance with regulatory requirements. Working closely with the Registered Manager, you will ensure that care is delivered safely, effectively, and with compassion.
Key Responsibilities
Service Coordination & Quality
Develop and maintain staff rotas to ensure safe staffing levels in line with the Health and Care Staffing Act 2019.
Monitor care delivery to ensure compliance with company policies, Care Inspectorate standards, and the Health and Social Care Standards.
Support the Registered Manager in implementing quality assurance measures and service improvements.
Maintain accurate records, including care plans, risk assessments, and electronic scheduling systems.
Customer Care
Act as a key point of contact for customers and families, ensuring clear communication and timely responses.
Support the creation and review of personalised care plans.
Promote customer dignity, privacy, and independence at all times.
Handle customer queries and complaints professionally, escalating where necessary.
Staff Support & Leadership
Assist with staff induction, training, and supervision.
Ensure staff are deployed effectively to meet customer needs.
Provide guidance and support to care staff, including responding to rota queries and emergencies.
Contribute to staff wellbeing and engagement initiatives.
Administration & Compliance
Maintain accurate scheduling and care delivery records using electronic systems (e.g., Nourish Care Planner).
Support compliance with safeguarding, infection control, and health & safety requirements.
Assist with audits, inspections, and reporting as required.
Ensure confidentiality is upheld in all aspects of work.
Essential Requirements
Strong organisational and administrative skills.
Excellent communication and interpersonal skills.
Experience in care coordination or frontline care delivery.
Knowledge of home-care services and regulatory responsibilities.
Ability to work flexibly, including participation in the on-call rota.
SVQ3 in Health and Social Care (or willingness to work towards).
Desirable Attributes
Experience using electronic care planning systems (e.g., Nourish Care Planner).
Train-the-trainer qualifications relevant to social care.
Experience in staff supervision and rota management.
Knowledge of business management in a care setting.
Additional Requirements
Enhanced Disclosure (PVG Scheme membership).
Full driving licence (maximum 6 points) and Class 1 business insurance.
