We are seeking a compassionate and dedicated Senior Support Worker to join our team. The ideal candidate will play a crucial role in providing high-quality holistic services to clients in their own home setting. This position requires a blend of leadership, communication, and IT skills to effectively implement care plans and enhance client's lives in their daily activities.
Responsibilities
Assist clients to remain independent in their own home providing holistic services.
Develop and implement individual care plans tailored to each client's needs.
Identify and escalate any concerns to ensure early intervention of health and wellbeing support to prevent hospital admissions and reduce isolation.
Supervise and support junior staff members, fostering a positive team environment.
Maintain accurate records of clients' progress and any changes in their condition.
Communicate effectively with clients, their families, and other healthcare professionals.
Ensure that the the clients home environment is safe, clean, and welcoming for all their family, visitors and friends.
Participate in training sessions to enhance skills and knowledge related to the senior support worker role.
Drive clients to appointments or social activities as required.
Requirements
Previous experience in the community or senior care setting is highly desirable.
Must have an NVQ 3 in health and social care or equivalent.
Proven supervisory experience with the ability to lead a small team effectively.
Strong communication skills, both verbal and written, with the ability to engage with clients compassionately.
Familiarity with IT systems for maintaining records and documentation is essential.
A valid driving licence is preferred for transporting clients when necessary.
Understanding of care plans and the ability to implement them effectively.
