The Role
We are seeking a proactive and knowledgeable Clinical Pharmacist to join our team. The successful candidate will play a pivotal role in enhancing patient care through expert medication management and support.

Key Responsibilities
Manage repeat prescriptions and ensure safe, cost-effective prescribing practices.
Conduct practice-based medication reviews for patients with chronic conditions. (LTCs) or complex health needs including vulnerable people
Work in the triage clinic and call in patients for assessment if indicated. If prescriber, sign their own prescriptions.
Review and action incoming hospital correspondence, particularly with regards to medicine reconciliation or initiation.
Manage medicines reconciliation post-discharge and implement safer prescribing practices.
Provide clinical advice on prescribing and medicines optimisation.
Support the practice in medicine safety (i.e. ECLIPSE alerts, MHRA alerts, drug shortages) and complete drug monitoring for high risk or specific named drugs or/and drug groups
Collaborate with GPs and other healthcare professionals to develop treatment plans.
Respond to medicine-related enquiries from GPs, practice staff, other healthcare teams (i.e. community pharmacy, hospitals) and patients.
Support the practice in achieving QOF and other performance targets.
Participate in audits and contribute to quality improvement initiatives.
Build strong relationships across health networks and with community and hospital pharmacists.
Person Specification
Essential:
Master's degree in Pharmacy (MPharm) or equivalent.
Registration with the General Pharmaceutical Council (GPhC).
Independent Prescriber qualification.
CPPE Primary care course Completed.
Minimum of 2 years' post-qualification experience.
Strong communication and interpersonal skills.
Ability to work independently and as part of a team.
Proficient in IT systems relevant to primary care.
Desirable:
Experience working within a GP practice or primary care setting.
Familiarity with clinical systems - EMIS Web
Experience in assessment and treatment of patients with Minor Illnesses.
Experience working in Hospital Pharmacy.
