About the Team




The Early Onset Dementia Team has been supporting younger adults and their families since 2000. Originally established to provide specialist assessment, day care, and outreach for individuals under 65 with a diagnosis of dementia, the team has evolved into a dynamic, integrated service offering comprehensive support throughout the diagnostic and post-diagnostic journey.


We provide specialist memory assessments, medication monitoring, and intensive support for individuals diagnosed with Alzheimer's, vascular dementia, frontal lobe dementia, or Lewy body dementia. Our team includes professionals from both health and social care backgrounds, with access to Occupational Therapy, Dietetics, and Psychology.

The Role




As a Senior Community Assessment Officer, you will play a key role in delivering statutory social work support. You will manage a caseload of patients, conduct memory and mental health assessments, and provide post-diagnostic care. You will also lead on Care Act assessments, safeguarding, and support planning, ensuring a seamless experience for patients and carers.

Key Responsibilities Include:



Clinical Duties:



Conduct home visits and complete ACE-III and mental health assessments. Manage caseloads through assessment, diagnosis, and discharge. Provide post-diagnostic support and crisis intervention. Maintain accurate records on System One. Participate in MDTs and make onward referrals to relevant services. Support complex cases and respond to changes in patient risk. Undertake duty work including triaging referrals and liaising with families.

Assesor Duties:



Provide advice on social care issues including carers assessments and financial support. Complete Care Act assessments and develop care/support plans. Conduct mental capacity assessments and carers assessments. Attend weekly MDTs and complex case discussions. Offer safeguarding advice and support to colleagues.

What We're Looking For



A qualified social worker or healthcare professional with experience in dementia care. Strong assessment and care planning skills. Excellent communication and multi-agency working abilities. A compassionate, person-centred approach to care.
