About the team



the Housing Solutions team offer specialist assessment across Hertfordshire for major housing adaptations. The team has a blend of community based Occupational Therapists and Community Care Officer Trusted Assessors who complete specialist housing needs assessments to identify major adaptation solutions to enable a person to have better access to the facilities within their homes. We explore personal strengths, their family and friends' support networks, and using expert knowledge we connect people with things that will help individuals and families to get on with their lives, achieving prevention, reducing or delaying the need for statutory provision of care services.



Housing Solutions is a county wide service and whilst we patch work within geographical areas, we may at times ask the team for flexibility to cover a wider area depending on the needs of the service.

About the role



Community Care Officer Trusted assessors will

:





Conduct holistic, person-centred assessments during home visits. Recommend both minor and major housing adaptations. Possess a thorough understanding of The Care Act and Housing Legislation. Connect individuals with local community resources. Operate in a preventative and enabling manner, fostering independence within homes and communities. Build strong relationships with district and borough council adaptations teams. Working on your own initiative as well as working as part of a team As part of your probation period, you will complete a level 4 accreditation of major home adaptations


About you

Essential:




An accredited RQF (registered qualification framework) level 4 social care qualification or equivalent.


An equivalent would be a course that has aligned learning outcomes to the RQF level 4 social care qualification and involves an extended period of study with a supervised practice placement in a health, education or social care setting.



If you do not hold such qualification, there is an expectation that you will work towards gaining this within the first year of being in post. The qualification is available via an apprenticeship route. Maths and English at level 2 (GCSE/O'level A-C/CSE grade 1) is a requirement.
Full driving licence Understanding of The Care Act and Housing Legislation Experience working in a social care setting

Previous experience minor or major home adaptations Completed functional assessment course of equipment provision

You'll need to have the ability to drive and/or travel around Hertfordshire, either using your own mode of transport or a company car to travel to other locations.

The ability to converse at ease with members of the public and provide advice in accurate spoken English is essential for the post (for those whose language is a signed language the provision of a sign language interpreter who speaks English to the necessary standard of fluency will be required).
