This is an exciting opportunity for any individual who is interested in palliative care and end of life care to join our dynamic specialist team.
We are seeking an enthusiastic and highly motivated Senior Healthcare Assistant to join the multi-disciplinary team in the provision of specialist palliative care to patients and their families.
The post will be integrated into the Specialist Palliative Community Hubs, and work alongside the Clinical Specialist team of Nurses and AHP to provide palliative care within the community.
The post holder will be an experienced Health Care Assistant with good understanding of end-of-life care and palliative care.
To manage a clinical Stable Phased (OACC) Caseload and take responsibility as key worker for individual patients, identifying and co-ordinating their specialist palliative care needs and referring to other team members when appropriate, working closely with the Clinical Nurse Specialist and Clinical Hub Leads.
To be responsible for holistic assessment, supportive mapping, advance care planning, and reviewing stable patients.
The post holder will need to be able to work autonomously and be responsible for holistic assessments and planning care to meet patient's practical, psychological, spiritual, and social needs. They will work closely with primary and secondary care professionals and Hospice colleagues, and good communication and team working skills are essential.
