Job Description

An exciting opportunity has arisen for an experienced Senior Community Mental Health Nurse to join the Assessment and Diagnosis team within the Dementia Assessment and Support Service.



This role will involve the assessment and support of service users in the community who have high level or complex clinical needs and will support the incoming referrals to the service via triage. The postholder will work within an MDT with Consultant input, but is mainly led by our Specialist Dementia Nurse Practitioners.



The service can be very fast paced and therefore the ability to work efficiently under pressure and respond to high service demand is essential. Additionally, the post holder will need to be confident to work autonomously and organise their time effectively. You will be highly motivated, have excellent communication skills and work well as part of a team, they will also have determination and drive committed to working holistically to improve the quality of life for people with dementia, their family and carers and also demonstrate a range of therapeutic interventions.



A good positive attitude towards this specialised client group needs to be demonstrated and also the aptitude to the contribution of continual development of the service incorporating evidence-based practice.



Take responsibility, for the management of a complex caseload



Responsible for participating in the setting of quality standards



Assess patient's individual care needs, develop, implement and evaluate programmes of care.



Deliver a range of interventions in accordance with the agreed plan of care and Pathway guidance including; pharmacological and psychologically informed interventions.



Complete and contribute, where appropriate to robust risk assessments and risk management plans. Deliver and promote positive risk taking, where appropriate, to improve quality of life and maximise occupational performance/independence.



Maintain an effective reporting system by observing and reporting verbally and in writing on patient conditions.



Utilise agreed outcome measures to review the effectiveness of interventions, as part of the care planning and evaluation process.



Participate in the development of the service to ensure clinical governance processes are actively implemented at team level.



Assist in ensuring that the aims and objectives of the ward/department (as set by the responsible nurse) are fulfilled and to identify factors which may inhibit these from being achieved.



The post holder will be required to work within the NMC Code of Practice and professional guidelines.



Maintain an up-to-date level of professional competence within the specific environment.
