Job Description

The post holder will:

Take responsibility for themselves and the quality of their work and the service they provide. Demonstrate expert clinical knowledge, health promotion and holistic assessment skills and will work with the patient and their carers to assess, plan, implement and evaluate their ongoing care and support needs in their own home or other community setting. Actively review, prioritise and manage the clinical caseload including triaging of new referrals over a seven-day working period. Participate in and undertake the annual staff appraisal and development reviews for more junior and support staff.

Actively contribute to the task of monitoring and evaluating standards of patient care, seeking to continuously improve the quality of care. Work in partnership with other key stakeholders including the Primary Care Network, local authority, care agencies and the charitable and voluntary sector.
