Secondment or Fixed Term Post
We are looking for an Occupational Therapist that values innovation and person -centred rehabilitation to join our supportive Community Rehabilitation Team. Keswick & Solway ICC Covers area of outstanding beauty from the Solway Plain to the Lake District National Park.
This post would be ideal for an experienced Band 5 wishing to gain community or Band 6 experience as well as experienced Band 6 staff
The team supports hospital discharge, works to prevent hospital admission, and offers community rehabilitation in Patient's homes or our established Rehabilitation Centre. The team also offers anticipatory care through Your Health matters and person centred support vis the Home Care Practitioner Service.
This role requires enthusiasm for holistic practice and utilising models of occupational therapy to assist individuals to achieve their occupational goals. Regular programme of In service training will help you to develop and to share your skills and expertise with others. An interest in personalised care handling is required but training can be provided

To undertake assessment, clinical and functional diagnosis of adults/older adults using a range of specialist standardised assessments and clinical observation skills across physical, sensory, cognitive, emotional, psychological and social domains.
To provide advice in the formulation or progress of a medical diagnosis and to support family members through this process guided by specialist theoretical knowledge and enabled by establishing a therapeutic alliance.
Contribute to the MDT's understanding of the family system and its impact on the patient, the patient's present circumstances, and the impact of that on the family system.
To formulate agreed specialist individual and/or group treatment plans and goals based on advanced clinical reasoning skills with the adult etc, taking in to account the domains assessed.
To provide specialist occupational therapy treatment interventions in a variety of environments
To provide spontaneous and planned specialist advice, teaching and instruction to others in, to promote understanding of the aims of occupational therapy and to ensure a consistent and coordinated approach across the AHP service.
Interventions regularly include the assessment, evaluation and prescription of specialised equipment for the adults. Standards for offering specialist advice for its safe use and the aims and purpose of the equipment apply as above.
Work in collaboration with other relevant services making appropriate onward referrals
