SENIOR FRAILTY PHARMACIST- COMMUNITY FRAILTY HUB Salary:
55,976 - 63,003 (WTE) Tenure: 2yr fixed contract (likely to become substantive) Hours: Full-time (Part-time hours will be considered on request) Location: York (with community-based/home visits) Job Summary: We are seeking an experienced and proactive Senior Frailty Pharmacist to join our multidisciplinary team within the Community Frailty Hub. This is a patient-facing role designed to deliver high-quality medicines optimisation, comprehensive geriatric assessments (CGA), and proactive frailty management. You will work alongside GPs, nurses, AHPs, and social care professionals to support complex frail patients both in their homes and in other community settings with a focus on safe, effective, and person-centred medication use. Your role will include medicines reconciliation, reviewing drug charts, deprescribing, and supporting safe transitions of care.
Main Duties and Responsibilities:
Key Responsibilities Clinical Duties Conduct structured medication reviews as part of CGA for frail patients living in the community. Support safe and effective deprescribing, ensuring medication regimens are aligned with goals of care, clinical appropriateness, and patient priorities. Undertake home visits to review and support patients with medication regimes, including compliance and use of MARS (Medication Administration Record Sheets) where applicable. Provide specialist input and advice to the MDT on drug optimisation for frailty syndromes (e.g.
falls, delirium, polypharmacy). Review and support the safe management of medications on Community Services drug charts. Identify patients at risk of medication-related harm and implement appropriate interventions. Liaise with community pharmacy, GPs, secondary care, and care providers to ensure continuity of care.
Leadership and Collaboration Act as a senior clinical member of the Community Frailty Hub, contributing to service development and quality improvement. Participate in MDT meetings, contributing clinical pharmacy expertise to decision-making. Supervise and support junior pharmacists and pharmacy technicians as appropriate in the future as this service grows. Quality Improvement & Governance Monitor and audit prescribing trends in frail populations to support evidence-based improvement.
Contribute to the development of local prescribing guidelines and pathways for frail patients. Support service evaluation and reporting for commissioners, contributing to the national frailty agenda.
