Job Description

This role offers a unique chance to deliver high-quality, compassionate care to patients admitted to our Frailty Ward. In Frailty we care for patients with a wide range of medical needs--from older adults with complex conditions to those requiring urgent assessment and treatment.



Whether helping with personal care, monitoring vital signs, or assisting with mobility and nutrition, your contribution will be essential to patient safety, dignity, and recovery.



You'll need strong clinical awareness, adaptability, and commitment to excellence. Responsibilities include collaborating with multidisciplinary teams, contributing to care planning, maintaining accurate documentation, and mentoring junior staff to foster learning and improvement.

As a senior HCSW you will:



Assist patients with personal care. This includes helping individuals with washing, dressing, toileting, and mobility, ensuring that each patient's physical needs are met with sensitivity and respect.

Support patients with nutritional needs, offering assistance with meals and hydration, and monitoring intake where necessary. These tasks are especially important in the Frailty Unit, where older adults may require more intensive support to maintain independence and prevent deterioration.



Monitoring and recording clinical observations is another key aspect of the role. SHCSWs are trained to take vital signs such as blood pressure, temperature, pulse, and oxygen saturation, and to report any concerning changes to the nursing or medical team
