Your role and responsibilities
As a Senior Independence and Wellbeing Practitioner, you'll play a vital role in reviewing the care and support needs of people receiving services. You'll ensure that support plans are person-centred, outcomes-focused, and of the highest quality.
On a typical day, you might:
Carry out assessments under the Care Act 2014 and review existing care packages.
Evaluate the quality and sustainability of commissioned services.
Assess mental capacity under the Mental Capacity Act 2005.
Complete Continuing Health Care checklists.
Conduct phone assessments, home visits, and attend professional meetings.
Work independently in the community, supported by a comprehensive induction and ongoing training.
You'll collaborate with social work colleagues, service development teams, and health partners to ensure joined-up, accessible services that promote independence and wellbeing.
