Your role and responsibilities
In this role, you'll work directly with people receiving services to review their care and support plans, ensuring they meet individual needs and promote independence and wellbeing. You'll be a key part of our mission to deliver high-quality, personalised outcomes that are economically sustainable.
On a typical day, you might:

apply safeguarding policies and procedures
review existing services to ensure quality and compliance
complete checklists under the Continuing Health Care Framework
conduct phone assessments, home visits, and attend professional meetings.
This role involves lone working in the community, and you'll be supported through a comprehensive induction, shadowing opportunities, and ongoing training.
What you will need
We hope you'll bring:
a warm, approachable manner
strong communication skills
confidence in a busy, target-focused environment
a commitment to learning and development
You'll work as part of a dynamic social work team, collaborating with colleagues in contracts and service development, and building strong partnerships across health and social care. You'll help ensure systems are joined up and accessible and promote Suffolk County Council's values in everything you do.

Co-production is central to our approach, and we're proud of our commitment to professional development and best practice.
Our work is guided by the People at the Heart of Care strategy and the Signs of Safety and Wellbeing practice model, promoting independence, personalisation, and digital care.
