We have an exciting opportunity for an Senior In-Reach Worker who has the skills and experience to assess and engage with our service users, ensuring that they access the right intervention at the right time, in a way that best meets their needs.

Where: Camden, London

When: We\xe2\x80\x99re looking to get the right person into the team ASAP

Hours: 37.5hrs per week

Salary: CGL Scale 28 - 30 (\xc2\xa330,316.74 - \xc2\xa332,412.09 dependent on experience, plus \xc2\xa33,915.44 Inner London Weighting.

Responsibilities

About the role:

• Provide bespoke interventions for identified service users, with the aim of engaging people in the most appropriate service for their needs, reducing re-presentations and bringing about positive client outcomes.
• Provide assessment and case management, including; care planning and time-limited interventions to support service users to make informed choices about their options.
• Working with service users to support social (re)integration, enabling them to lead meaningful and purposeful lives: promoting recovery, resilience, peer support and self-determination.
• To work closely with all partner services, sharing appropriate information and ensuring that action plans are carried through.
• To maintain accurate and comprehensive service user records in line with Change Grow Live\'s procedures and protocols and the requirements of the local authority, maintaining confidentiality and sharing information with appropriate agencies.

About you:

• Have a strong understanding of substance misuse issues and experience of working within a similar field
• Be an excellent communicator, both verbal and written with good IT skills
• Proactive and able to work on own initiative with excellent time management and prioritising skills
• Have a good working knowledge of mental health interventions, services, and good practice
• Be a supportive team player with strong interpersonal skills with the ability to work in partnership with a wide range of professionals, agencies, and internal/external stakeholders

• Ability to manage change successfully in a way that prioritises the needs of service users
• Understand the importance of information governance processes and commit to follow and apply all necessary safeguards
• Seek out learning opportunities to improve and broaden your professional knowledge and skills and to contribute and oversee the learning and development of others
