A rare and exciting opportunity for a Senior Community Nurse has arisen within our West Suffolk Community team for adults with learning disabilities and autism.



As a team we continually strive to develop the services we offer, to ensure they're high quality and evidence based. To support our aims, we are seeking enthusiastic and highly motivated nurse to join our dynamic multi- disciplinary service. You'll be joining an innovative team of nurses, working collaboratively with multiple agencies and professional clinicians.



Our service covers the geographical area of West Suffolk and you'll be responsible for managing a caseload from our base in Bury St Edmunds.



It is the ideal time to join us; the way we care for people with Learning Disabilities in Suffolk is changing at NSFT as we have recently benefitted from 1.3 million of investment for our Acute services and the introduction of a new Learning Disability and Autism Specialist Service.



The development is fully driven by the national Transforming Care Agenda. The Learning Disability and Autism Specialist Service is not only being invested in financially, with a fresh approach to ideas and service delivery being supported by the Trust, we are confident our new model of care is the very best for the people we care for.

The ideal candidate should possess excellent and varied communication skills, enjoy working as part of a team, and working closely with colleagues from a range of disciplines. You will be able to work independently, carry out comprehensive assessments and plan, implement and evaluate delivery of nursing interventions as part of the holistic approach.



You will also be able to demonstrate that you are able to apply your nursing skills to a wide range of situations, as you'll be working with a range of adults with a learning disability living within the community.



We have a culture of continued learning, working as a close LD specialist pathway with each team having monthly protected days which includes a variety of training, reflection, supervision and service development.

We'll support and encourage you to develop specialist experience and expertise as appropriate - whether you wish to be part of QI projects or be part of a leadership group planning and implementing the driving change for Transforming Care and Quality Standards for Learning Disability. All applicants will either be registered on Part 5 or 14 of the NMC Register.



Please note, you must have a full and valid driving licence due to the community element of the role and the large geographical area that the West Suffolk Community Team cover.



The team is based in Bury St Edmunds and there will be an expectation of travel. Due to the nature of community health services, the successful candidate will have a clean driving licence and access to a vehicle for work purposes.

CPD is determined through appraisal with a personal development plan agreed between an individual and their manager, with the commitment of the necessary time and resources.
