Job overview




Memory Assessment and Treatment in Norwich and North Norfolk.





Working with people who have experienced changes in cognition and memory and may be developing dementia. The team provides a comprehensive memory assessment and treatment service and works with a variety of other services to connect people diagnosed with dementia to the support they need.



You will be based with the Central Locality Memory Assessment Team and assess people with cognitive and memory changes and have a risk of developing dementia. The team provides accurate diagnoses and formulates holistic, person-centred care and treatment. Much of the work you undertake will be in a clinic setting at the Sophie Centre, Julian Hospital, Norwich.



Our Team is on a developmental pathway; aiming to become accredited with the Memory Services National Accreditation Programme. We are committed to developing and improving the service. We value research and have received a Trust Excellence Award for our engagement in research.


Main duties of the job




The post holder will assess people with changes to memory and cognition and work towards a clinical diagnosis and appropriate treatment and care plan. They will have support from an experienced team that includes Consultant Psychiatrists, Clinical Psychologists, Senior Nurses and an Occupational Therapist.



NSFT's Community Services for Older People were rated Good in the most recent CQC inspection report and we are proud of our positive and caring approach to the patients and carers we are working with.



As a team we focus on Continuing Professional Development and have a drive for further developing our clinical knowledge and skills. The team have established links with Primary Care and other agencies, which enhance our work and ensure we are able to help and support our patients.



The team operate 9-5 hrs between Monday to Friday.

Detailed job description and main responsibilities




The post holder will undertake the planning and delivery of direct patient care with appropriate supervision, ensuring continuity of practice and working within a multi-disciplinary and multiagency environment. Support and advising to other health care professionals. To support aspects of service development. Provide an appropriate learning environment for relevant students.

Qualification




Essential criteria




Registered Mental Health Nurse with the relevant professional registration Practice Supervisor / Practice Assessor training (in line with NMC Standards for Student Supervision and Assessment 2018)

Desirable criteria




Evidence at social care / health related study #

Experience

Experience of working within a multidisciplinary team Familiar with risk assessment and its management
