What You'll Do
Manage a caseload of clients with mental health diagnoses, supporting them through complex and challenging circumstances.
Provide one-to-one, person-centred support in clients' homes and community settings, travelling across Berkshire to meet their needs.
Build supportive, trusting relationships with clients, fostering a positive and empowering atmosphere where progress can flourish.
Enable clients to access relevant services and support networks, both within Browns Community Services and through established external partners.
Facilitate clients' integration into their communities, helping to reduce isolation and encourage meaningful engagement.
Work collaboratively with external stakeholders, complementing their interventions to ensure the best possible outcomes for clients.
Guide clients through the next steps of their personal progression journeys, helping them to sustain engagement with vital services and overcome barriers to success.
Support clients to understand and manage their symptoms, empowering them to actively participate in their treatment and recovery plans.
Attend probation and court appointments alongside clients with forensic histories, ensuring compliance with legal requirements and maintaining support if clients enter prison or secure hospitals.
Assist clients in claiming benefits and attending essential meetings, advocating for their rights and wellbeing.
Help clients secure and maintain stable tenancies, promoting independence and long-term stability.
Establish productive relationships with partner organisations to continually enhance service outcomes.
Monitor and record client progress using our Progression Charts database, contributing to ongoing improvement and accountability.
Co-create realistic, achievable plans with clients, breaking down goals into supported, manageable steps.
Develop strong networks with local organisations, representing Browns in meetings and events to raise the profile and reach of our service.
Support the Project Manager by preparing monthly and quarterly reports as required, ensuring high standards of service delivery.
Show initiative, enthusiasm, and flexibility, working independently and as part of a team to provide the best possible support for our clients.

Requirements & Skills
Applicants must have NVQ2 qualification in Health care or equivalent qualification/experience
Minimum of 3 years' experience of supporting clients with complex needs out in the community.
A full driving license and use of a car is essential for this role - paid travel across Berkshire.

If required for your post, undertake an enhanced DBS check and maintain annual membership through the update service ADDITIONAL RESPONSIBILITIES
Adheres to all the policies and procedures of the organisation Commits to Continual Professional Development and undertake any training and development deemed necessary to fulfil criteria of post.
