Reading Intermediate Care?- Senior Physiotherapist (Band 6)



Location -Reading Intermediate Care,7-9 Cremyll Road, Reading Berkshire RG1 8NQ



The post holder is a Senior Physiotherapist (band 6) with the clinical skills, expertise and knowledge to support community reablement, community Physio &?Falls service.



Intermediate care is a multidisciplinary service that helps people to be as independent as possible. It provides support and rehabilitation to people at risk of hospital admission or who have been in hospital. It aims to ensure people transfer from hospital to the community in a timely way and to prevent unnecessary admissions to hospitals and residential care.



Reading Intermediate Care is a team of physiotherapists, occupational therapists, a nurse, and senior therapy assistants, who treat people in their own homes to rehabilitate them to as high a level of independence as possible. This supportive multidisciplinary?team works with patients across Reading in close communication with social services, voluntary agencies, and GPs.?This post involves 7 days working within a shift pattern within the intermediate service.



Complete the timely assessment and management of referrals with multiply co-morbidities and complex rehabilitation needs .



To support triage the referrals for intermediate care services



Provision of equipment to promote independence and safety.



Hold a caseload and work autonomously



Work flexibly to ensure urgent referrals are adequately managed



Supporting colleagues in the delivery of compassionate, safe, and structured care



Provide guidance and appropriate delegation within the team



To participate in service development and quality improvement



To support the supervision of more junior members of staff and provision of education and clinical physiotherapy advice to staff /students in Intermediate Care

Current understanding of intermediate care with experience of rehabilitation and hospital discharge



Car driver with ability and readiness to be flexible within working day
