Are you looking for a challenging new role? Do you have a desire to work within a fast-paced crisis network? We have an exciting opportunity for a Senior Practitioner to join the Home Treatment Team service



The Sheffield Home Treatment Team are pivotal for our recent transformation supporting our service users in the community and gatekeeping the bedded units . The new post holder will play an important & integral role in operationalising the new model of service.



The Home Treatment team is a 7 day service including Bank Holidays, which is split geographically into 4 sectors inline with the PCMHT and CMHT

The post holder shall work closely with 3 other senior practitioners to provide leadership, clinical expertise and supervisory support and provide cross cover as required.



The HTT support people who are experiencing an acute mental health crisis by providing intensive support in their own home as an alternative to hospital admission. The HTT approach is both step down and step up utilizing gatekeeping skills and least restrictive practice



HTT provide acute mental health intervention to service users in Sheffield , primarily supporting people aged 18 and over.



The MDT comprises of Psychiatrists, ANP's, Nurses, Support Workers and other professionals.



This is not an exhaustive list of duties and responsibilities and in discussion with the manager the postholder may be required to undertake other duties which fall within the Banding of the post. The job description will be reviewed regularly in the light of changing service requirements and any such changes will be discussed with the post holder.



Provide a clinical/practice overview and input into the team. The post holder will provide a senior clinical and day to day managerial leadership role within the service. The post holder will be an integral part of the leadership/ management structure, developing practice, practitioners & team functioning.

Principal Duties



Responsible with the Service Manager and other members of the Leadership for managing and developing staff from a range of professions by providing regular work supervision and appraisals.



Be a positive role model to promote a conducive working environment.



Responsible with the Service Manager and Leadership Team for developing effective team working.



Responsible for the development of evidence based practice within the service.



Responsible with the Service Manager and Leadership team for ensuring that the service adheres to Care Pathways and service wide systems and processes for the delivery of health and social care



Responsible for the development of service policies and protocols in conjunction with other senior practitioners and managers across the service.



Responsible with the Service Manager for ensuring effective systems and governance structures to support the effective delivery of care.



Other Duties



Deputise for Service Manager in their absence ensuring continuity of service and maintenance of standards.



Take responsibility for ensuring own professional competence.



Ensure that behaviour and conduct comply with SHSC's policies and procedures eg Health and Safety, Equal Opportunities, Confidentiality and Data Protection.



Adhere to professional codes of conduct.



Professional and Advisory Role



Advise others of the role and function of the service.



Provide specific clinical and professional advice to a broad range of professionals.



Maintain close working relationships with professional leads.



Develop and maintain a professional interface with other services.



Establish and develop positive partnerships with primary care professionals and related community mental health services.



Develop and maintain clinical standards within the service in keeping with both local and national policy.



Service Development



Responsible with the Service Manager and others in the Leadership team for auditing practice within the team.



Responsible with the Service Manager and others in the Leadership team for ensuring that staff undertake appropriate training.



To maintain contemporary knowledge of educational developments in relation to the service and profession.



Routinely engage with service improvement initiatives.
