Are you a Qualified Mental Health Nurse, Social Worker or Occupational Therapist with urgent care experience that is looking to progress and lead on the development of our CMHT?
An exciting opportunity has arisen within Trafford CMHT to work as a Senior Practitioner within a well established Homebased treatment team. As a senior role, there will also be a requirement to supervise and support staff members within the CMHT in their professional development, alongside participation in service development projects within the team .
The post holder will, amongst other duties, -
Deputise for CMHT team manager in their absence providing cover for Zoning, MDT's and other professional meetings .
Assist with supervision of junior members of staff and provide regular IPDR reviews, including following HR guidance around performance.
Assist with taking the lead and advising on safeguarding concerns within the team.
Engage in the locality on call manager rota.
Proactively engage in Senior leadership meetings to ensure KPI and trust targets are achieved.
Engage in service development projects to enhance care delivery in CMHT.
Ensure high standards of clinical contact with service users and carers accessing the CMHT.
Assist with overseeing and reviewing the CMHT Unallocated caseload to ensure service user flow through the CMHT.
Provide guidance and advice to practitioners dealing with complex service user issues such as funding applications, attending panel, completing COP/DOLS applications, completing capacity assessments, completing Nursing Needs and Care Act Assessments.
Review and manage complaints, incident reviews and learning events.
Provide senior cover for CMHT Duty referrals, providing advice and guidance to ensure correct outcome for service users.
