Our team provides essential support for adult service users, ensuring safe and timely discharges from hospital. Working closely with Peterborough City Hospitals multi-disciplinary teams, you will enable discharges within the Discharge to Assess framework.

Excellent knowledge of current legislation alongside strong motivational skills. The ability to be able to work and support the team whilst also being able to make informed decisions utiising your own judgement.
