We have an exciting opportunity for a Senior Primary Care Nurse to join our Primary Care team at HMP Moorland, a men's category C Prison and young offender institution located in South Yorkshire. Free parking at all sites, low turnover of staff at these sites, wellbeing days, strong relationships and partnerships.
Hours
37.5 hours a week working Monday to Sunday - 7.30am-7.30pm - Working 3 or 4 days a week, 9 or 11 hour shifts weekdays and 12 hour shift on weekends. Some nights 7.30pm-7.30am / 7.30pm-5.30am. Every other weekend but could be 1 in 3.
Salary
This position is full time, youll receive an annual salary up to 47,466 per annum FTE - depending on experience.

Main duties of the job
As a Senior Primary Care Nurse, your responsibilities will vary. You will

• Lead a multi-disciplinary team to deliver a nurse led service and prevent hospital admissions
• Undertake medication rounds, dispense controlled medication, attend emergency response situations, assess patients, and undertake reception screening
• Provide outstanding evidence based care to our patients, seeking out, listening to and acting on their feedback, so that care is personalised and informed by what matters to them.
• Deliver non-judgemental care that makes a real difference
• Inspire excellence by forging strong links with our partners and stakeholders and providing a service that people can trust, feel safe within, and feel proud of
• Promote best practice, clinical supervision, evidence based care and continuous quality improvement, embedding a culture of shared learning.
• Share knowledge, skills and expertise to ensure safe care, building strong multi-disciplinary teams and supporting each other to do a great job.

Qualifications required:

• Registered nurse Adult
• Current professional qualification (NMC)
• Long term conditions experience would be beneficial
• Evidence of Continued Professional Development
• Understanding of the complexities of multi-disciplinary and multi-agency working
• Excellent communication skills
• Proactive problem solving skills
• Motivated as an individual and when working in a team
