A position has become available within the Clinical Pharmacist team at the Southend Hospital site.

The post will involve rotations within varied specialist clinical areas including general medicine, surgery, orthopaedics, department of medicine for the elderly and technical services, as well as contributing to the provision of the 'on-call' service.

Deliver clinical pharmacy services to wards, outpatients, discharge teams, the Technical Services Unit, and other areas.
Attend ward rounds and multidisciplinary meetings as appropriate.
Record clinical interventions and activities accurately.
Provide specialist medicines advice to medical, nursing, and other Trust staff.
Promote safe, effective, and cost-efficient use of medicines.
Counsel patients on the correct use of their medications.
Participate in the emergency on-call pharmacist rota, working independently to provide out-of-hours advice, supply, and sourcing of urgent medicines.
Support the Medication Safety Officer and Pharmacy Governance Pharmacist with medicines optimisation and risk management.
Promptly report clinical incidents (e.g., dispensing or administration errors) in line with Trust procedures.
Ensure safe storage of medicines within pharmacy and ward areas.
Support implementation of local and national guidelines (e.g., NICE).
Maintain accurate medication and supply records using the Pharmacy WellSky system.
Supervise trainee pharmacists, students, and ward-based technicians.
