About Us

At

Positive Impact Care Group

, we're not here to tick boxes -- we're here to transform care.

We believe in doing things the right way: leading with values, building services around people, and backing our teams to grow, learn, and thrive.

We're expanding our outreach services across

The Wirral and Liverpool City Region

, and we're now looking for a passionate and motivated

Outreach Support Manager

to help shape and lead this exciting area of growth.

What Outreach Means to Us

Outreach support is about empowering people to live their best life -- on their terms.

You'll be supporting adults with

learning disabilities, autism, and complex needs

to live independently in their own homes and communities.
That might mean supporting someone to plan their week, manage daily tasks, get out into their community, maintain relationships, or learn new skills that build confidence and independence.

This isn't about "care hours" -- it's about

real outcomes

: building lives that are meaningful, connected, and fulfilling.

No institutions. No one-size-fits-all. Just genuine, person-centred support that fits around each individual.

The Opportunity

This is a leadership role with

purpose and impact

.
You'll be at the heart of developing a growing outreach service, leading a skilled team, and shaping how support is delivered across the region.

You'll also have the opportunity to develop your own leadership journey, with clear progression as the service expands.

What You'll Be Doing

As

Outreach Support Manager

, you'll play both a strategic and hands-on leadership role, ensuring safe, effective, person-centred care delivery.

You'll:
? Lead, motivate, and support outreach teams to deliver consistent, high-quality care
? Manage referrals, assessments, and transitions with commissioners and social workers
? Oversee safe staffing levels, rota planning, and service delivery across multiple packages
? Ensure full compliance with

CQC regulations

and safeguarding requirements
? Build strong relationships with families, professionals, and internal teams
? Monitor quality, outcomes, and performance data to continually improve services
? Contribute to growth and service development within the Liverpool City Region
? Embed our

TEAMS values

throughout your leadership:

Transparency, Engagement, Authenticity, Motivation, Sustainability

You'll be supported day-to-day by our

Registered Manager

and wider leadership team, with regular check-ins, supervision, and professional development opportunities.

What We're Looking For

We're looking for someone who's:
- Experienced in supported living or community-based care services (minimum 2 years leadership preferred)
- Confident in CQC compliance, safeguarding, and quality standards
- Organised, adaptable, and proactive in managing changing priorities
- A natural communicator -- supportive, assertive, and always people-focused
- Passionate about person-led support and building services that make a difference

Most importantly, someone who wants to

see the impact

of their work every day -- and feel proud of it.

What You'll Get from Us

We believe people do their best work when they feel valued.
