About the Role



We are looking for a committed and empathetic Short Breaks Assessor to join our Children's Services team. In this role, you will assess the needs of children and young people with disabilities and their families, ensuring they have access to appropriate short break services that promote inclusion, independence, and well-being.




You will work closely with families, providers, and partner agencies to develop tailored support plans that meet individual needs and comply with statutory requirements.


Key Responsibilities



Conduct assessments for children and young people requiring short breaks. Develop and review support plans in partnership with families and professionals. Ensure services are delivered in line with safeguarding and regulatory standards. Provide advice and guidance to families about available short break options. Maintain accurate records and contribute to service improvement initiatives.

About You



Experience in social care, family support, or a similar role. Strong understanding of the needs of children with disabilities and their families. Excellent communication and interpersonal skills. Ability to work collaboratively with families and multi-agency partners. Knowledge of safeguarding principles and relevant legislation.
