Denbighshire's Single Point of Access Team are looking for an enthusiastic, organised individual with keen listening skills and an interest in ensuring the citizens of Denbighshire are supported to manage their well-being and independence.

Denbighshire's Single Point of Access (SPoA) aims to provide an effectively coordinated single point of access, single point of assessment and care co-ordination for preventative and rehabilitation work for adults across Denbighshire. The SPoA acts as the 'front door to services' to ensure our citizens receive the Right Care, in the Right Place by people who have the Right Skills at the Right Time - First Time! We are committed to enabling people to have greater choice and control over their lives to enable them to live as independently as possible.

The post holder will be part of the team who act as the first point of contact within the Single Point of Access. They will provide Information, Advice and Assistance (IAA) to the people of Denbighshire, by developing a broad knowledge base about health and well-being services provided by Local Authority, Health and the Third and Independent Sector in order to signpost citizens appropriately, empowering them and enabling informed choice.

The ability to communicate in Welsh would be highly advantageous to any candidate applying for this post.
