We are a part of Children's Services in Education & Learning, and we work very closely with the Children & Families Trust, Adult Social Care and other key partner agencies, as well as parents and carers of children and young people with SEND. We are looking for a Social Care Assessor to join our dedicated team working with the Designated Social Care Officer, in the SEND Service. This role is an opportunity to make a real difference in our vibrant and diverse city, in a creative and dynamic team working to improve outcomes for children and young people with SEND.
Role Description
To directly contribute to achieving the aims of the Service including:

• To research records relating to a child or young person who may be known to social care and share this information with Education Colleagues as part of the EHC Planning Processes.

• To offer and undertake assessments to identify if a child requires a social care service as part of the EHCA Assessment processes.

• To work in partnership with other professionals involved with the children and families to deliver tailored support plans using legal frameworks appropriate policies

The main responsibilities of this role * To offer and undertake assessments of need to identify if the family requires access to a service for SEND.

• To undertake direct work with children/parents/carers/extended family as part of a SEND Assessment Processes.
• To work effectively with partner agencies to support a holistic approach to meeting the needs of children with SEND as champions of Social Work Services.
• To signpost to appropriate interventions that promote the engagement of children, parents and extended family.
• To ensure the appropriate participation of children and young people in their own assessment, plans and reviews and to respond appropriately to non-engagement.
• When identifying concerns about a child welfare, to report those concerns to the team's manager and children's safeguarding teams as appropriate.
• To ensure that polices are followed and their responsibilities are fulfilled to take actions to safeguard vulnerable children, young people and adults.
• To support and contribute to evaluation, auditing and quality assurance including gaining the views and experiences of service users of current service delivery and future expectations.
• To ensure that services are delivered in compliance with relevant legislation, local and national policy and guidance.
• To participate in supervision and appraisal of own practice and in training and group learning activity.
• To ensure that all key information is contained within the SEND Portal.
• To ensure that records are kept in accordance with the policies, procedures and standards of the department.
• To maintain and develop communications and co-operation with colleagues, individuals and groups in the community and other statutory and voluntary agencies.
