Social Care Coordinator - Continuing Healthcare (CHC)

We have established team for Continuing Healthcare assessments, in partnership with our Health partners. North Yorkshire Councils Continuing Healthcare Team is managed by a dedicated Team Manager, and as we recognise the value of multi-professional teams, the team includes an Advanced Practitioner, Social Workers, Social Care Coordinators and an Occupational Therapist.
You will be instrumental in supporting improved outcomes for people who require assessments for Continuing Healthcare. As a Social Care Coordinator in this team, you will support assessments and decision making regarding Continuing Healthcare eligibility, acting as part of a multidisciplinary team to complete the Decision Support Tool. You will also be responsible for case management where there are joint package arrangements between social care and health, completing timely reviews to ensure the persons needs and outcomes are met appropriately.
You will become a specialist in the field of Continuing Healthcare, supporting people and their families through the assessment process and sharing your skills and knowledge with the wider Health and Adults Services Directorate.
We would love to hear from you if you have previous experience of completing assessments under the Care Act 2014, which is essential to this role. We also welcome applicants who have experience in the completion of the NHS Continuing Healthcare Checklist tool and being part of a multi-disciplinary team to complete the Decision Support Tool.
We value the specialist knowledge this team has, and we support the team with regular peer support and specific practice sessions so there are lots of opportunities to support your continued professional development as well as to ensure you have peers available to seek advice and support from.
We have established strong relationships with our NHS partners and we work jointly to create the right pathways and processes for the team.
The team is demonstrating a real difference, and we have shown we can improve the outcomes for people who receive our support. The team will continue to have a key role in the development and shaping the future for the Continuing Healthcare Service within NYC.
The team works alongside our NHS colleagues Monday to Thursday 08.30 to 17:00 & Friday 08:30 to 16:30, and we offer flexibility within the hours of work which can be discussed and considered. At present the team is not required to work weekends but this may be subject to change in the future should the service require it to do so.
We have various offices available throughout North Yorkshire, so wherever you are currently located, we can work with you to organise the most practical office base for you as well as working from home to form a hybrid model of working.
