Social Care Practitioner Adults
Job Description
The successful candidate will need to evidence that they have experience in understanding and supporting the needs of adults with physical disabilities and older people.
This post will provide an excellent opportunity for a Social Care Practitioner to develop their skills and knowledge within a friendly and supportive team. You will be responsible for assessing and arranging support for adults virtually, at our local hubs and in people's own homes in order to promote their independence. Excellent organisational and IT skills and an ability to work at a fast pace and under pressure are key requirements for this role.
