Job Description
We are currently recruiting a Social Care Practitioners to join our Community Area Teams and Acute Hospital Team Adult Social. We have full and part time positions available, in the area teams there is a level of hybrid and face to face visits. In the acute hospitals there is a need to be onsite and working as a multi-disciplinary team with our health colleagues.
