Social Care Practitioner
Job Description
A vacancy for an enthusiastic and well-motivated Social Care Practitioner has arisen in the North community Social Work team based in Market Drayton. The successful applicant will need to evidence that they have some experience in understanding and supporting the needs of older people, adults with learning disabilities and adults with physical disabilities.
The post-holder will be responsible for assessing and arranging support for adults via the internet, at our local hubs and in people's own homes in order to promote their independence. An ability to work at a fast pace and under pressure, excellent organisational and IT skills are essential requirements.
