Responsibilities of a Social Care Worker:



Assessing Needs:

Carrying out assessments of individuals' physical, emotional, and social needs to develop appropriate care plans.

Creating and Reviewing Care Plans:

Developing, implementing, and regularly reviewing personalised care plans based on each service user's unique circumstances.

Liaising with Other Professionals:

Working closely with healthcare professionals, social workers, families, and local authorities to coordinate effective care and support.

Safeguarding and Advocacy:

Ensuring the safety and well-being of vulnerable individuals by reporting concerns and advocating for their rights and needs.

Providing Emotional and Practical Support:

Offering guidance and assistance with accessing services such as housing, benefits, or mental health support.

Record Keeping and Reporting:

Maintaining accurate records of care plans, assessments, progress, and incidents in line with regulatory requirements.

Monitoring and Evaluation:

Regularly evaluating service users' progress and making necessary adjustments to care plans and interventions.

Promoting Independence and Inclusion:

Supporting individuals to make their own decisions where possible and encouraging active participation in the community.

Understanding Legislation and Policies:

Ensuring all actions comply with current legislation, including the Care Act 2014, Mental Capacity Act 2005, and GDPR.

Qualifications



Minimum NVQ Level 2 in Health and Social Care (or willingness to work towards it) Registered with SWE (Social work England) A recognised degree in social care Work or equivalent qualification. Registration with the relevant professional body is essential. Previous experience in a social care work setting is highly desirable. Strong interpersonal skills with the ability to build rapport with clients from diverse backgrounds. Excellent organisational skills and attention to detail. Ability to work independently as well as part of a team. Experience in supervising or mentoring others is an advantage. A commitment to continuous professional development.
