Social Care Worker - WMF2498e

----------------------------------

Job description

Location: Across Westmorland and Furness Council

Contract Type: Fixed Term

Hours 28 hours per week

About this opportunity

We have an exciting opportunity, for a passionate, enthusiastic person to come and join our dedicated and driven team, during an interesting and ambitious programme of change within Adults Social Care, to help us develop and deliver our goals for the future.

We want you to become an integral part of the implementation phase of our Older Adults and Physical Disabilities team across Westmorland and Furness, supporting people aged 18 who are living in the community and have been identified as having longer term care and support needs.

As a social care worker within the Older Adults and Physical Disabilities team you will work within the team undertaking strengths-based assessments and delivering appropriate person-centred support and outcomes for people in Westmorland and Furness, who have care and support needs. The service works collaboratively with a range of stakeholders from across the health and social care system and third sector and voluntary groups in Cumbria, in ways that recognise strengths, promotes independence and prevents, reduces or delays the need for formal long term care and support provision. Assessments and reviews will be undertaken in a strengths-based way that maximises the use of social capital and community based assets. The Service works across health, social care and third sector organisations with a primary focus upon ensuring that all support plans:-

Include robust contingency plans that promote personal resilience to manage temporary changes in need/circumstance; Demonstrate how people will meet their needs from a range of sources, including friends, family, third sector, the community, equipment or assistive technology. Personal budgets will only be allocated for the provision of formal support for identified unmet needs.

Who we are looking for...

We are looking for a compassionate and motivated individual with experience or awareness of social care, healthcare, or the voluntary sector. You should be empathic and have strong communication skills to collaborate with customers, carers, and stakeholders. A proactive approach to risk management, and health and safety is essential, along with the ability to plan workloads, meet deadlines, and use IT effectively. Most importantly, you should be committed to promoting independence, influencing positive change, and ensuring high-quality, strengths based care.

What our team says-

'I am an ASYE Social worker, working in Eden OAPD, for Westmorland and Furness Council. On my first day, I arrived in the office, feeling very nervous to embark on this new journey, and I was immediately put at ease by the warm welcome the team gave me. Throughout my time with the team, they have been incredibly supportive. As a newly qualified social worker, I have always felt that someone is always there to ask a question, when I have been unsure. This has been invaluable for me, as this has facilitated my learning and helped my confidence grow. The team are very friendly, knowledgeable, professional, caring, and welcoming, it's a great team to work in.' Amy - ASYE Social Worker.
