We have an exciting opportunity for a Social Prescriber to join our Primary Care team at HMP Stocken, a

category C

Prison located in

Oakham.





HMP Stocken is a busy male prison with a capacity for 1300 inmates. We are recruiting for a Social Prescriber to support the wider Healthcare Team with 1-1's with patients, supporting the patients with alternative therapies such as art or wellbeing activities. We are welcoming applications from newly qualified candidates.



HMP Stocken offers free parking, free on site gym. There is also a Bistro on site for which you can have subsidised meals. Not on a direct bus route so would need your own transport.

Hours





0900 - 1700
This role is two days a week, you can choose which two days to work between Monday - Friday.

Salary





This position is part time, you'll receive an annual salary up to 33,510per annum FTE - depending on experience. The salary would be pro rata'd for the part time hours on offer.

What you'll be doing




As a Social Prescriber, your responsibilities will vary. You will
Be building professional relationships with the people you are helping by listening carefully to what is important to them and what motivates them. You will be responsible for connecting people to activities and services within the prison, and required to support and motivate patients to access opportunities to help improve their health and wellbeing. This role will see you liaising with key healthcare staff, building and maintaining relationships with a range of prison departments, charities and third sector providers to promote social prescribing and ensure social prescription opportunities are available within your sites.
