Job Summary


The Social Prescriber plays a vital role in connecting individuals with community resources and support services to improve their overall wellbeing. This position involves working collaboratively with healthcare professionals and community organisations to address social determinants of health. The Social Prescriber will engage with patients, assess their needs, and provide tailored support to help them navigate available resources effectively.

Responsibilities



Provide one-to-one personalised support to patients, helping them identify issues affecting their health and wellbeing. Develop tailored action plans with patients to improve their confidence, independence, and ability to manage their own health. Connect patients to local community groups, services, and support networks for issues like housing, debt, loneliness, or lifestyle changes. Maintain accurate, confidential records and contribute to service monitoring and evaluation. Work closely with GPs, nurses, care coordinators, and voluntary sector partners as part of an integrated care approach.

Essential:



Excellent communication and active listening skills. Ability to build trusting relationships with a wide range of people. Strong organisational and time-management abilities. A flexible, patient-centred approach. Knowledge of local community services and the voluntary sector (or willingness to learn).

Desirable:



Previous experience in health, social care, or community support roles. Understanding of health inequalities and social determinants of health. Basic IT skills and confidence using clinical systems (training provided).

Pay: 26,000.00-28,000.00 per year
