Frimley ICB is one of 15 pilot areas across England to be part of a programme to rollout the Department for Work and Pensions (DWP) and Department for Health and Social Care (DHSC) plan to support people with health conditions back to work. The pilot will do this by supporting people to develop sustainable strategies to manage their conditions, develop self-care plans and have access to peer support networks to support ongoing recovery and wellbeing.
The WWSP will be vital to the project's success in the Rushmoor District. In order to achieve the project indicators, they will conduct either telephone or face to face assessments. These will identify the key barriers to employment and provide support and advice to overcome those barriers working with Citizens Advice, who will handle the employment elements of the project. They will support a caseload of people whose barriers to employment may be because of physical health, mental health and/or social situations. Using the Work Well pathway work plan, the post holder will provide support, advice or signpost and refer to more appropriate external services.
This post as advertised as a fixed term contract to end of March 2026 initially,. However we hope the project will be successful and will be ongoing.
WorkWell Participants:
People who are currently working but struggling to remain active in their role because of their health condition. They may be experiencing periodic episodes of sickness which they or their employer feels are getting worse.
People who are employed but currently off sick and would benefit from support & a minor intervention to facilitate a return to work.
People who have recently fallen out of employment and are looking to return to the workplace perhaps on a different career path as soon as possible.
Eligibility:
Participant's home address OR address of their GP/local Jobcentre Plus must fall within Rushmoor Borough or Hart District.
Participants must be over 16.
Participants must have the right to work in the UK.
Participants have a health-related barrier to work.
WorkWell Project Delivery:
The WWSP will provide 8 hours of contact with the participant over 8 weeks. This will include:
Identifying and confirming eligibility of participants.
Develop and support a personalised WorkWell plan with the participant.
Signposting to other more appropriate and supportive services.
Work with the Citizens Advice, for support with employment queries.
Support completion of participant experience and satisfaction reviews.
Take part in the external WorkWell evaluation.
Work with the JOY app, to support the participant and also record data for each participant. Complete and submit the monthly return to the ICB and DWP.
*JOY is a case management system for Social Prescribers.
Main Responsibilities:
1. The WWSP will receive and process referrals directly from Primary Care colleagues with the PCN the post holder is based in.
2. Receive referrals from Job Centre Plus and Employers and work with Citizens Advice to ensure the most appropriate support is put in place.
3. Validate participant's eligibility prior to offering support.
4. Manage a caseload of participants who have common mental, physical and social health problems or long-term conditions.
5. Carry out assessments to identify barriers and develop action plans in agreement with the individual to overcome those barriers
6. Provide support on a 1:1 basis to support the delivery of the participants plan,
7. Consider all options and refer to other services where additional support beyond the 1:1 support is required. This may include some physical activity, alternative means of transport - this list is not exhaustive.
8. Work with external organisations as needed to support the participants journey through the WorkWell pathway.
9. As the WorkWell pathway develops, consider the option of group sessions for participants where there is common ground and enable some peer support and a more efficient way of working
10. To maintain detailed and accurate case notes, action plans and WorkWell participant paperwork and upload these on to the management information system within the allocated timeframe.
11. Ensure all information relating to the participants is uploaded to the JOY app.
12. To collect and record specified management information, as determined by the line manager, and respond to requests for management information from service commissioners and DWP.
13. Support participants to upload the getUbetter App (if appropriate to participant). The App can support participants to recover, live and work well with common musculoskeletal injuries and conditions and also women's pelvic health.
14. Support training with colleagues from external organisations to help raise awareness and improve their knowledge about this pilot and the WorkWell model, as well as supporting them to understand the common barriers and LTCs.
15. To travel across a reasonable area with agreement, to deliver services to participants who are unable to access the service at the normal base.
Along with your main duties, you will also be expected to carry out any other duties that are reasonably asked of you.
Training Requirements:
There will be some essential on-line training required (JOY, getUbetter for example) as well as the RVS standard induction training before the WWSP can fully engage with the project. This will be arranged & facilitated once the WWSP is appointed.

About You:
You will be educated to a good standard, be administratively self-supporting and possess excellent IT skills. You will possess a positive, 'can do' manner, be approachable and respectful with the ability to motivate and influence people.
You will have experience of project/service co-ordination and delivery, ideally within the voluntary sector.
The role is subject to a basic DBS check.
