Social Prescribing Link Worker Bidford Health Centre - Part time 7.5 hours per week

Bidford Health Centre, located near the semi - rural village of Bidford on Avon and within easy driving distance of Stratford-upon-Avon, Alcester and Evesham for example and is a large, modern and purpose built health centre incorporating a dispensary and is a teaching practice for GPs, nurses , Physician Associates and medical students. We have a list size of 14,250 patients and an exciting future.

We have a strong team of 4 Partners, 5 Salaried GPs, Nursing team of 7 and multi-disciplinary team of clinical and non-clinical staff.

Main Duties and Responsibilities



Social Prescribing is a free service that supports people to improve their health and wellbeing in a holistic way by connecting them with non-medical community resources.

As a Social Prescribing Link Worker, you will:

Take referrals from GPs, place-based teams and other partner organisations Build strong relationships across health, social care, voluntary and community sectors Provide personalised, non-clinical support to individuals and their families Support access to community services such as: Advice and information services Community and voluntary groups Leisure and sporting activities Lunch clubs and self-help groups Specialist interest groups

Essential Criteria



Experience supporting people, their families or carers (paid or unpaid) Strong listening skills with a person-centred, non-judgemental approach Ability to work with people from diverse backgrounds Commitment to professional and personal development Ability to work within policies and procedures, including confidentiality, safeguarding, lone working, information governance, and health and safety Personal accountability, emotional resilience and ability to work under pressure NVQ Level 3 or equivalent qualification (working towards or willing to work towards)
We are an award winning teaching practice for a variety of roles and happy to support and develop an enthusiastic individual.

Accessing the health centre is easy by car with plentiful parking and a bus service outside running between local towns. We expect the role to be practice / hybrid based as an integral part of our MDT.

A DBS assessment and usual checks will be conducted as part of the recruitment process. You will benefit from a good holiday entitlement, access to the NHS pension and an excellent working environment.

Pay: From 31,049.00 per year
