involve Community Services is working together with the New Wokingham Road and Woosehill Care Network (PCN) to deliver targeted, proactive social prescription. This will motivate, engage and connect patients, assisting them to make improvements to their health & wellbeing. This intervention will help develop greater independence, create resilience and reduce the demand/ dependency on primary care.
This PCN includes two GP surgeries: Woosehill and New Wokingham Road (Crowthorne).
Main Role Tasks and Responsibilities
As a key member of the PCN multi-disciplinary team, you will provide personalised support to patients, their families and carers in order to improve health and wellbeing.
Build strong links with the multi-disciplinary teams within each GP surgery.
Accrue and make best use of local knowledge regarding local health, social care and voluntary provision to support patient care and independence.
Offer referred patients one to one support, either remotely or in person , building boundaried, non-judgmental and impactful connections.
Utilise a strength-based approach focusing on a person's assets.
Following initial assessment, create personalised plans that focus on improving health and wellbeing and show the outcomes the patient wishes to achieve.
Proactively encourage volunteering opportunities and access to local community assets, enabling peer to peer support, good neighbourliness and 'active citizenship'.
Introduce and if required, accompany patients to groups, clubs, and meetings they would like to try.
Where applicable, help patients to gain access to other forms of assistance such as that offered by utility companies, the fire & rescue service and benevolent funds.
Seek advice and support from the GP, your involve Wellbeing Services Manager and/or other identified individuals to discuss relevant patient-related concern e.g. safeguarding concerns or support with mental health.
Ensure an empowerment model is employed to reduce patient service dependence.
Keep clear and comprehensive electronic records, inputting all related patient data onto the involve Community Services and the associated surgery's case management system.
Monitor and track any gaps in service provision or any unmet needs and proactively discuss this in supervision.
Monitor and track patient progress.
Ensure patients are given opportunity to offer feedback and are aware of our complaints policy.
Act at all times in a professional, fair and inclusive way treating all patients equally.
Undertake continual personal and professional development in accordance with your role.
Adhere to organisational policies and procedures, including confidentiality, safeguarding, lone working, information governance, equality, diversity and inclusion training and health and safety.

Undertake any other tasks or duties that may reasonably be required in relation to this role.
