About the role
Please note that the advertised salary for this position is subject to a pending pay award. The final salary will be adjusted in line with the nationally agreed pay award.





We are currently looking to recruit a permanent Social Work Assistant to join Porth Ceredigion integrated intake team, to work with individuals and their family's within hospital and community settings with a focus on recovery and rehabilitation. This is an exciting role within the service providing the opportunity to undertake assessments and implement packages of care to support our organization's aim to enable citizens and family resilience.





The role is one of constructing and implementing Care and Support Plans that are person centered and outcome focused that responsive and flexibly to changing needs.





Porth Ceredigion is the integrated decision making hub for Ceredigion that triages and determines the level of need of all cases and the best outcomes for those individuals. Porth Ceredigion is the heart of service provision that focuses on ensuring that Citizens receive the intervention that is best placed to meet their needs or where else to guide them to early help or specialist services.





What Porth Gofal provides:


A responsive service to Citizens living in Ceredigion. Applies a Strength based needs assessment focusing on service user choice and control using the Signs of Safety framework. A reduction in the time Social Care professionals spend in determining eligibility for statutory services. Legislatively compliant service that meets the statutory requirements of the SSWBA. A safe service that identifies vulnerable and at risk Citizens, that responds effectively.

What Porth Gofal achieves:


Provides information, advice and assistance about community health and social care services for adults that improves the connection between people and their communities. Encourages a balanced approach to the assessment of need ensuring that citizens are at the center of conversations, decisions and the "what matters" conversation takes place. Multidisciplinary triaging of referrals to identify the right professional to respond e.g. physiotherapist, Occupational Therapist or Social care workers to ensure right response at the right time. Provide more consistent, coordinated assessment for care and support. Improve the flow of information between services, resulting in faster decisions being made about people's needs and support. Reduces hospital admissions. Provides an in-reach service to reduce delayed transfers of care by providing timely discharges from hospital.

A commitment to integrated working with a focus on positive rehabilitation outcomes for the citizens is essential.





The ability to travel throughout the county is essential.





We are looking to recruit an individual:


HNC/Nursing Qualification/NVQ4 in Care/QCF National Diploma Health & Social Care Level 5, K101 or equivalent. Knowledge of preventative and reablement approaches that improve independence. Knowledge of care assessment and support planning. A bilingual policy is in operation and candidates who do not have the linguistic ability in Welsh will be supported and encouraged to achieve the ALTE standard within 2 years.
