About the role




Southern Hospice Group are looking to appoint a qualified social worker to provide a social work service to families (children, young people and adults) under the care of Chestnut Tree House comprising:


Holistic assessments of children and families, using established frameworks. The provision of ongoing emotional support pre- and post-bereavement. The provision of resource information advice and advocacy for families to have improved access to help them access services from other health, education and social care providers. Promoting well-being and resilience, thereby supporting family members to continue to meet the needs of their life-shortened child and reducing the risk of family breakdown due to families having access to specialist support when needed. Work alongside statutory colleagues to keep children and young people safe.

Essential criteria




Social work qualification and registered with Social Work England. Experience of working in a health care or local authority setting and/or of working with children with disabilities. Experience of multi-professional working. Evidence of well-developed communication skills. Evidence of an ability to work with individuals and families who are experiencing significant loss. Evidence of an ability to work with people of different ages and backgrounds, including children and young people. Strong communication and listening skills to support children and families with sensitivity and compassion. Understanding of child development, safeguarding, and the emotional impact of life-limiting conditions. Knowledge of social care systems, palliative care principles, and relevant legislation. Ability to assess family needs, advocate for support, and work closely with a multi-disciplinary team. Emotional resilience, empathy, and adaptability to manage challenging and sensitive situations.

The role may be emotionally demanding due to the nature of the role and the clients with whom the post-holder will be working.

Personal grief must be resolved sufficiently to perform and cope in the palliative care setting.
