Job Description


We are very excited to be able to offer a substantive Social Worker post within the Eating Disorders Service. The post is part time 22.5hours per week; days/ hours can be negotiated and we are seeking an experienced Social Worker. The post would be based at the Brownhill Clinic/Eastgate House, a community base for the community eating disorder teams to enable lots of helpful information/ skills/ knowledge sharing and positive interface working.
The Eating Disorders Service is a friendly, welcoming and dynamic team providing secondary care services to children and young people, adults of working age, their families and significant others, to provide the best possible outcomes for people experiencing an eating disorder. We strive to ensure that service represents excellent quality, (safe and a positive experience) and good value, (efficient and effective).
This is an exciting opportunity for a Social Worker who has some experience of working with adults of Working Age and caring for individuals with an Eating Disorder and other forms of Mental Illness.
Main duties of the job
The post holder will:
Work with allocated individuals with an Eating Disorder who have social care needs and serious and enduring mental health difficulties.
Develop and provide social care/health evidence-based interventions for individuals of working age with an Eating Disorder who are receiving social care/health funding and managed through the Social Care Hub.
Ensure a clear focus on recovery and social inclusion
The role will entail assessment, support planning and review of social care packages in robust and timely manner, in accordance with The Care Act 2014 requirements.

Job responsibilities
Manage the service provision for a defined caseload of individuals with an Eating Disorder who have social care and health needs.
To act as Care Manager where appropriate.
To ensure appropriate risk assessments are completed.
To ensure all child protection, adult safeguarding and Mental Capacity Act issues are dealt with according to policy.
To respond to requests for assessments and review of need for people with social care needs. This will be under the framework of the Care Act 2014 or the statutory duties arising under s117 of the Mental Health Act.
Working in partnership with service users, carers and other professionals as agreed within the multi-disciplinary team and with the Eating Disorders Service including out of county/area reviews.
To work in partnership with service users, carers, other team members and other statutory and non-statutory agencies.
To liaise closely with Primary Health Services and other agencies at the point of discharge to ensure a smooth transition.
To participate in professional supervision and workload management on a regular basis as required by the team manager and social care specialist.
To undertake referrals to the carers hub as appropriate.
To maintain and promote a recovery focus in all interventions with a particular emphasis on social inclusion and meaningful occupation.
To maintain contemporaneous health and social care records and comply with relevant Trust polices.
To ensure that all relevant data is entered onto Trust information systems (e.g. RiO, Social Care IT systems) in a timely manner.
To positively address issues of discrimination and social exclusion.
To use the results of and participate in clinical audit and research as required.
