An exciting opportunity has arisen to recruit a Social Worker to the Community Team for People with Learning Disabilities (CTPLD) in North Somerset.

About the role

This is an exciting and challenging time for Adult Care Services, where integration is key to meeting the aspirations of people with a learning disability. The CTPLD is a multi-disciplinary team whose members include Social Workers, Psychologists, Psychiatrist, Community Nurses, Physiotherapists, Occupational Therapists and the Intensive Support Team.

The position is available on a part time (22.2hrs/3 days per week), permanent basis based in Clevedon and supporting the surrounding areas. We operate a hybrid way of working which enables you to work from home when best suited to your planned day.

We are seeking applicants who hold a Social Work Qualification with experience of working with Adults - preferably within a community setting. Applicants need to be registered with Social Work England, ideally with experience in Care Management (Assessment and Review), Safeguarding Adults and Mental Capacity Act processes. We also welcome applications from newly qualified social workers.

This post is exempt under the provisions of the Rehabilitations of Offenders Act (Exceptions) Order 1975 and is subject to an enhanced Disclosure and Barring Service check.

Valid driving licence and own transport, or be able to provide alternative, suitable method of travel is an essential condition for this role.

Your role will include;

  • Undertaking assessments, reviews and working with service users to maximise their independence.
  • Completing Mental Capacity assessments and Best Interest processes.
  • Supporting the safeguarding of vulnerable adults and work with other key professionals/families to draw up risk management plans.

Consultation is currently underway to determine the new location for colleagues based in Castlewood; This will be at Weston-super-Mare or within North Somerset.
