Warrington are on an exciting journey shaping the future of intermediate care services. We are currently looking for an Experienced Social worker to join our Intermediate Tier Services in Warrington.
This is a fantastic opportunity to be a part of shaping integrated care working with partners to improve outcomes and experiences for the people of Warrington. We are looking for a value-led, dynamic, outcome driven and compassionate individual to join the service at a time of significant opportunity. We are well on our journey to delivering collaboration across Health and Social Care to improve outcomes for those living with frailty in Warrington.
Our intermediate care services are made up of a range of services including:
Rapid Response Services in addition to our well-established Care Call services we are also one of seven nationally acclaimed accelerator sites for NHS England's Aging Well program, delivering a new urgent community response service to people experiencing a health or social care crisis. Our highly skilled team is made up of health and social care professionals and carers working together to support people at home or in the community and in doing so contributes to the avoidance of hospital admission or an admission to respite care.
Intermediate Care at Home Services, where we provide a wide range of care, social work, rehabilitation, therapy, equipment and assistive technology services, supporting people to live well and independently either at home or in the community. We take pride in our GOOD CQC registration for our care services and our re-ablement outcomes are strong with over 80% of people receiving our service remaining independent at home for three months after leaving our service.
Hospital Discharge Team and Intermediate Care Bed Bases where we support the safe and timely transfer from hospital into settings that best meet individual needs, providing care and rehabilitation to regain independence and undertake assessments for their long-term needs.
The role
To work within the Councils statutory duties with regards to adults with care and support needs utilising a strengths-based approach to assessment and support planning. Assess and manage areas of risk, need and service provision, To ensure the safeguarding of adults and children with care and support needs is inherent within practice. Contribute to a team to support continuous development and operational effectiveness.
What's needed?
Recognised social work qualification
SWE registration
Evidence of Continuous Professional Development
Knowledge of the equipment and adaptations criteria, assessment process and provision
Knowledge of relevant legislation, Government policy and guidance in relation to social care, health and housing
Full and current driving licence
Enhanced DBS clearance
