We are looking to appoint a qualified Social Worker to work in our Integrated Team within the department of Communities providing a full range of social work duties including assessing older adults with physical disabilities and older adults with cognitive impairment in the community and providing a full range of care management services in line with the Social Services Wellbeing Wales Act 2014.


You must be motivated and be able to work in a demanding environment across health and Social Care with knowledge of community and hospital setting.


The post is based in the South of the County in Llanelli but travelling across the County will be required.

Progression beyond pay point 40,221, up to the maximum of 44,428 is subject to a progression panel assessment under the Authority's 'Social Work Experienced Practitioner Progression Framework'*


Experienced Practitioner Progression Framework


The 'Framework' recognises Social Workers who have completed their consolidation programme, and who routinely undertake complex social work practice. It also recognises those Social Workers who undertake relevant post qualifying, CPD programmes and research, which could include being a Practice Educator who has 2nd and 3rd year Students, or an AMHP licenced and practicing within the Authority.
