About the Role:



Lambeth is an exciting place to work with lots of transition happening. If you are looking for a challenging and rewarding opportunity to work in a complex team within Adult Social Care Community teams, then look no further!


As a social worker within the Physical Disabilities and Older Persons (OPPD) Care Management Team, you will be dealing with complex OPPD cases including, Court of Protection applications, community reviews, adult safeguarding enquiries and so on; liaising with colleagues in Health and commissioning to facilitate discharges from step down beds, and meeting the challenges in ensuring delivery of high-quality service to our local residents.


In order to maximise resources and achieve customer focused outcomes, you will need to have excellent communication, organisation and planning skills and be committed to partnership working across statutory and third-sector organisations and working as a part of the team to ensure performance targets are met.
