We are looking for 2 social workers to come and join our Local Mental Health Teams (LMHTs). One will be based at LMHT Gedling (1.0 FTE) and the other in at LMHT City East (0.6 FTE).
There is an exciting opportunity to join the growing social work workforce within Nottinghamshire Healthcare NHS Trust. Social workers play a crucial part in improving mental health outcomes for individuals, through understanding the social detriments of mental health and using a human rights approach. This requires advanced relationship-based skills focussed on personalisation and strength-based working, to support people to make positive, self-directed change.
The successful candidates will work as a social worker in a multi-disciplinary community mental health team. This involves supporting a caseload of individuals' who are experiencing social issues which are impacting their mental health by applying social work and psycoeducation interventions and representing the social work and social perspective in the team. Social work training and supervision is provided as well as opportunities to develop additional intervention skills required for working in a community mental health team.
The post holder will demonstrate the principles of social work through professional judgement, decision making and actions within a framework of professional accountability. They will promote and lead on the social perspective and trauma-informed social model of mental health and wellbeing within the multidisciplinary team. They post holder will value and take account of the expertise of service users, carers and other professionals.
The post holder will work within a multi-disciplinary team providing social work expertise to improve mental health outcomes for individuals through understanding the social determinants of mental health. This includes completing pieces of work where a patient requires social work input/ social work interventions; and providing care coordination for people experiencing social stressors that are impacting their mental health or social stressors are the predominant issue that requires support.
The post holder will also complete team responsibilities as required.

The post holder will maintain an individual caseload and complete team responsibilities.
The post holder will ensure that all individuals are assessed holistically considering the individuals' health and social needs to ensure personalised support planning. This will involve engaging with the individuals' family, carers, and support network when appropriate.
The post holder will assess and analyse risk/ ongoing risks to ensure personalised risk and safety plans, balancing risk management and safeguarding with choice, self-determination and positive risk-taking underpinned by relevant legislation.
The post holder will focus on strength/ asset-based working, to support people to make positive self-directed change within their lives and engage with their communities.
The post holder will recognise the risk indicators of different forms of abuse and neglect impacting individuals and provide expertise to the multidisciplinary team with regards to adult and children's safeguarding responsibilities. This includes sharing disclosures appropriate, management of sensitive information and public protection. They will take on the safeguarding link role in the team. The post holder will ensure evidence-based working informed by social work interventions, theories, and values. They will also ensure a human rights approach with knowledge of the statutory rights of individuals.
The post holder will promote family and carers work within the multidisciplinary team and link in with the Family Intervention Team. They will train and complete relevant interventions (with other colleagues) to enable this work within the team.
The post holder will maintain and provide expertise to the multidisciplinary team with regards to application of legislation including the Care Act, Mental Capacity Act, Equality Act, Human Rights Act, Transforming Care, Mental Health Act and other legislation relevant to the remit of the team.
The post holder will be responsible for working in partnership with agencies external to the Trust supporting adults and carers to access services/support in the statutory, independent, and non-statutory sector.
The post holder will liaise and work with Social Care to support the individual to access statutory social support and ensure those open to social care are receiving the correct level of support. They will also support colleagues within the multidisciplinary team to understand and engage with Social Care and relevant processes.
The post holder will model anti-discriminatory and anti-racist practice, challenging system impacts.
The post holder will work proactively as an effective member of the multi-disciplinary team and attend team meetings as required.
The post holder will promote a learning culture and share and disseminate best mental health social work practice within the multi-disciplinary teams.
The post holder will provide supervision to colleagues as required and appropriate.
The post holder may occasionally be required to deliver medication. They will consult with the prescriber and follow relevant procedures.
The post holder will undertake any other duties that would be a reasonable expectation of this role including working as part of the duty system.
A full UK driving licence and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.
Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. The cost of the DBS application is 26.40 (standard) or 54.40 (enhanced), this cost will be deducted from your salary over the first 2 months of employment.
