An opportunity for the post of social worker has arisen for the SCOPE team which is a specialist team working with individuals aged 14+ going through school transitions; whilst also supporting individuals across their whole life span. The individuals we support have a diagnosed learning disability and/or autism and are identified as having complex care needs.



The SCOPE Team is a Health and Social Care multi-disciplinary team. SCOPE stands for:




S - Supporting young people and adults with complex needs

C - Community based approach/assessment

O - Offering young people and adults' choice in their care packages

P - Person-centred planning

E - Enriching people's lives.



The principal role of the SCOPE team is to provide life-long support for complex cases of individuals who reside across Perth and Kinross. The team is made up from disciplines from both Health and Social Care, such as social workers, Outreach Workers, Psychology and aligned to NHS services that include Speech and Language Therapist, Physiotherapy, Dietetics, Psychiatry and Community Learning Disability Nurses. The team will link with local partners such as Education and Children's services, Housing, other Health Services and third sector partners.



SCOPE will be leading in developing locally based provision, which will improve the lives of those with complex care requirements as well as their families and carers. The team will have a whole person approach and will embed the principles of positive behavioural support into the delivery of provision.



Social workers within the team follow a hybrid model working between home, the community and Pullar House, with newly qualified workers expected to use Pullar House as their main base.



The social workers in the team are responsible for transition planning alongside outcome focussed assessments and reviews; the social workers undertake specialist and person-centred care planning through the Self-directed Support (Scotland) Act. Whilst also undertaking work in relation to the Adults With Incapacity (Scotland) Act and Adult Support and Protection (Scotland) Act.

Skills & Experience Required

We are looking for an individual who has a qualification in social work to undergraduate, diploma or masters level.



The successful candidate would ideally have experience of working with children, young people or adults with specialist, complex care needs who also have a diagnosed Learning Disability and/or Autism; and who also have an understanding of taking a person centred approach to care planning and an understanding of positive behavioural support for those who experience stressed and distressed behaviours.



You will have an ability to undertake robust assessments and care planning which will enable you to update care plans to ensure the outcomes of the individuals are being met. As well as having the flexibility and skills to respond to changing needs of individuals. You will be able to demonstrate experience of multi-disciplinary working and be committed to working closely with colleagues from other disciplines within the team.



You must be able to work effectively within a team, as well as be able to work independently and autonomously to meet deadlines and demonstrate a commitment to improving service provision.



You will have good organisational skills and be able to manage a caseload. Excellent verbal and written skills are essential.



You must also have a valid UK driving licence as this post requires you to travel to a variety of locations across Perth and Kinross.
