Job Responsibilities

Provide support and guidance to individuals and families in need, helping them access services and resources. Conduct assessments to identify the needs of clients and develop personalized care plans. Collaborate with healthcare professionals, community organizations, and other stakeholders to coordinate care and services. Maintain accurate and up-to-date records of client interactions and progress. Advocate for the rights and needs of clients within the community and with service providers. Participate in case conferences and team meetings to discuss client progress and plan interventions. Provide crisis intervention and support during emergencies or critical situations.

Person Specifications

Relevant qualification in social work or related field. Registration with a recognized professional body. Proven experience in a similar role, preferably within a local government or community setting. Strong communication and interpersonal skills, with the ability to build rapport with clients and stakeholders. Excellent organizational skills and the ability to manage a diverse caseload. Empathy, patience, and a genuine desire to help others. Ability to work independently and as part of a team. Knowledge of relevant legislation and policies affecting social work practice. Proficiency in using computer systems and software for case management and reporting.
