We are seeking a dedicated and compassionate Social Worker to join our Learning Disabilities and Neurodiversity Team. This role is pivotal in providing support and advocacy for individuals with learning disabilities and neurodiverse conditions, ensuring they receive the care and services they need to lead fulfilling lives.

Key Responsibilities:
Assessment and Planning: Conduct comprehensive assessments of individuals' needs and develop personalised care plans in collaboration with service users, their families, and other professionals.
Support and Advocacy: Provide ongoing support and advocacy for individuals, ensuring their voices are heard and their rights are upheld.
Collaboration: Work closely with a multidisciplinary team, including healthcare professionals, educators, and community organisations, to coordinate and deliver holistic care.
Crisis Intervention: Respond to crises and emergencies, providing immediate support and developing strategies to manage and mitigate future risks.
Record Keeping: Maintain accurate and up-to-date records of all interactions, assessments, and care plans in accordance with legal and organisational standards.

Qualifications and Experience:
Education: A degree in Social Work and registration with the relevant professional body.
Experience: Previous experience working with individuals with learning disabilities and neurodiverse conditions is highly desirable.
Skills: Strong communication, empathy, and problem-solving skills. Ability to work independently and as part of a team.
