The OPMHT is an Integrated Community Team including both Social Services and Health staff. As a Social Worker within the team, you will work closely with other Professionals to negotiate outcomes with service users and carers and identify solutions to Achieve them. In accordance with legislation namely Social Services and Well-being Act 2014, Mental Capacity Act 2005, Mental Health Act 1983.


The post is a

Older Person's Mental Health Team are committed to: -

Ensuring all health and social care needs and risks are assessed and that service users are well managed and safe from harm. Providing services that are accessible to all service users within secondary mental health services in compliance with equality and diversity principles. Working with service users within a model of care that assists in promoting positive outcomes and enables them to return to live safely within the community. Working collaboratively with a range of professionals, users and carers as well as non-statutory services. Assessing the needs of carers and ensuring appropriate support is provided. Actively involving service users and carers in planning and delivering services to their needs. Promoting the needs of people with mental health related needs and reducing the stigma associated with mental health care.
